Physical and Health History Update Forms

The Physical and Health History Update Forms are still hard copy forms that need to be filled out and turned into the Nurses' Office. For a Physical or Health History Update to be approved by the first day of try-outs/practices it must be submitted to the Nurse's Office. The Physical Form and Health History Update Forms can be printed by clicking on the prior links. Physical Forms can be handed in at the High School Main Office Monday through Friday from 8:00am to 2:00pm. It is recommended that all candidates make a copy of their physical prior to handing it in. Physicals must be completed within 365 days prior to the start of the sport. All Physicals must completed on the NJ Department of Education Pre-Participation Physical Evaluation Form (PPE).

  • 2019 Fall Sports by Friday, July 26, 2019 
  • 2019-2020 Winter Sport Physicals by Friday, October 11, 2019 
  • 2020 Spring Sport Physicals by Friday, January 31, 2020  

The Pre-Participation Physical Evaluation Form (PPE) may ONLY be completed by a licensed physician, advanced practice nurse (APN) or physician assistant (PA) that has completed the Student-Athlete Cardiac Assessment professional development module.  It is recommended that you verify that your medical provider has completed this module before scheduling an appointment for a PPE.

Before you leave the physician’s office, the following check list should be reviewed to ensure that your physical is complete. Our school physician will not approve your physical unless all pages are completed correctly.

Parent Section - History Form & Supplemental History Form
  • Student demographic information is complete
  • Medicine & Allergy information is complete
  • All questions are answered on the history form                                
  • YES answers are explained completely, where indicated, on both pages
  • Athlete & Parent signature and date are completed on both pages
Physician Section - Physical Examination Form & Clearance Form
  • ALL Physical Findings (height, weight, BP, pulse and  VISION acuity-not prescription) are filled out by physician. Vision may not say “Declined, Sees Eye Doctor, or Done in school”
  • Clearance, Physician signature & date of exam are completed on both pages
  • Health Care Provider Office Stamp is placed in box
  • Physician signature/date indicating completion of the Student-Athlete Cardiac Assessment Professional Development Module

The physical will be returned to you if all the information is not complete. 


Physical & Health History Update Forms are currently available and can be printed out. The Physical and Health History Update Forms can be dropped off at the High School Main Office.

All Athletic Department forms required to participate in High School Interscholastic Sports are now online (excluding Physical & Health History Update Forms).

2019-2020 Winter Sports on Friday, October 11, 2019 at 9:00am 
2020 Spring Sports on Friday, January 31, 2020 at 9:00am.

  • Register as an individual and create your account
  • On the Top menu bar select “Find Organization”
  • Search For Bridgewater-Raritan and Click On “School Name”
  • Click on the “Sport Season” you wish to register for
  • Click the appropriate sport, and fill in all relevant questions. (any question with a red star is required to complete registration)
  • When finished click “Submit” located in lower right hand corner

No one may participate in try-outs/practices without a Physical that has been approved by the School Physician. 

The ImPACT Test should not be taken unless instructed to do so by the Athletic Training Staff. Your son or daughter will be informed by the Athletic Trainers if they are required to take the ImPACT Test. Please do not do so unless directed.
